As stated on dnellejOy's deviantart, its a mod from the HomeMatte theme by Lauro.

Yep, I found it, I realise now they are notification icons, I was wondering what they were!

you might need to rename it (bottombarknobgray.png)

crispaso's shot uses neutraface font...can't help you with that.

Think its called Slock. Zack Schiller (Ithink thats how you spell his last name) made it. He created Rhomer and Neco, so if you go to those threads you shold find a link to his site and that lockscreen.
