How can you have only three columns? Is this only with photoshop or have you the same on your iPhone?

You can install Five Icon Dock, FIve-Column SpringBoard, and iBlank. Then place 2 blank webclips (using iBlank) in each row of your SB.
You wont get the exact same icon locations as the preview, but it will be a start.
